Job Fair with NSO Authentication & PRC License Renewal

JOB FAIR WITH NSO AUTHENTICATION, PRC LICENSE RENEWAL AND LICENSURE EXAMINATION FOR TEACHERS (LET) REGISTRATION
MAY 26, 2012, 8:00am to 3:00pm
Provincial Training Center (Formerly GSP Bldg.) 
Mankilam, Tagum City

REQUIREMENTS:
PRC Renewal
1. Duly accomplished renewal form
2. Two (2) pcs. Passport size picture: close-up, colored, plain white background, with complete name tag;
3. Photo/Xerox copy of recent professional ID card;
4. In case of lost profession ID card which is still current, the applicants shall submit a notarized affidavit of loss;
5. In case of destroyed professional ID card, the applicant shall surrender the destroyed card.
6. Renewal fee is P 420.00.

PRC LET Application

(For first timers)

1. Transcript of Record with scanned picture with remarks for Board Exam purposes;
2. Special Order (S.O.) of the applicants;
3. Original Birth Certificate (NSO SECPA) and bring photocopy
4. Original Marriage Contract (NSO SECPA), LCR validated and bring photocopy
5. Two (2) pcs. Passport size picture: close-up, colored, plain white background, with complete name tag;
6. Residence Certificate (Cedula)
7. Application Fee is P 900.00

(For repeaters)
1. Two (2) pcs. Passport size picture: close-up, colored, plain white background, with complete name tag;
2. Notice of Admission (if still Available);
3. Residence Certificate (Cedula)

NSO
1. Fill up the application form
2. Payment of required fee
3. Note: Authorization and ID of the document owner together with the requester’s ID are required if the requester is NOT any of the following:
a. The owner of the document
b. His/her parent
c. His/her spouse;
d. His/her direct descendant
e. His/her legal guardian/institution-in-charge, if minor
